Medical Director:
-
Certifies that patients meet the medical criteria for hospice admission
-
Provides oversight of physician services
-
Supervising all hospice physician employees and contract hospice physicians;
-
Assuring overall continuity of the hospice medical services
-
Assuring that the patient receives appropriate measures to control uncomfortable symptoms.
RN Case Manager/Nurse:
-
Determines patient eligibility,
-
Address physical issues & helps patient remain free from discomfort
-
Provides education of patient’s illness
-
Maintains communication with patient’s physician, assists with health decisions, etc.
-
Utilization of Durable Medical Equipment only
Hospice Aide:
-
Assists under the supervision of a RN with patient’s personal care
-
Bathing, hair care, shaving, skin care, linen changes, catheter care, etc.
Medical Social Worker & Bereavement Coordinator
-
Provides emotional support & guidance
-
Identifies community resources
-
Assists with planning for changes
-
Offers information about advance directives, etc.
-
Provides support to families for 13 months following the patient’s death:
-
Periodic mailings
-
Connection with grief support services
-
Referrals to community resources, etc.
Spiritual Adviser
-
Provides spiritual support based on respect of patient/family’s personal faith & beliefs
-
Regular visits & assistance with linking patients to their faith community, if desired.
Volunteers:
-
May provide many different types of support when needed:
companionship, telling stories, running errands, holding a hand, etc.
